While we accept most PPO insurances, we recommend you check with your insurance company prior to scheduling an appointment to verify coverage.

Form/Letter Policy

There is an administrative fee to cover expenses related to completing forms and writing letters, such as those for school, sports or medical accommodations. These tasks require time and resources from both the staff and providers and reflect a service that is not covered by insurance companies.

Fees:

Forms (including physicals, camps, sports, travel and more) $25.00

Letters of medical necessity $25.00

Same day fee (in addition to above) $25.00

Legal forms/documents (1 page) $50.00

Legal documents (multiple pages) $100.00

All paperwork requires a 48 hour minimum turnaround time
We are unable to complete or generate any recommendation letters or forms regarding family law or legal matters without a subpoena or other official court document.
