New Patient Checklist

Excellent care happens when the physicians and office staff are aligned with your family's priorities and philosophies regarding medical care. Only then can shared decision-making be based on mutual respect.  Please read the following checklist carefully to ensure Conejo Children's Medical Group is the appropriate medical home for your children.  We look forward to meeting you and forming a mutually rewarding relationship.

  • Please provide your child's past medical records (or newborn records) and vaccine records prior to your first visit.  Our providers review this information to prepare for our first to make it as smooth as possible.
  • Health insurance issues often cause billing issues.  It is your responsibility to confirm CCMG is covered by your insurance plan prior to scheduling a visit to avoid unexpected costs to your family. 
  • Please bring a photo ID and a physical copy of your current insurance card to each office visit.  If your insurance company only provides a digital copy, you may request a physical card from them or create your own to provide at the time of service.

  • Vaccines

    CCMG believes in the safety and effectiveness of recommended vaccines and does not advise following alternative immunization schedules.  Our Vaccine Policy follows the schedule outlined by the American Academy of Pediatrics (AAP).


     We are willing to work with your family regarding your questions and concerns about immunizations.  However, if you are planning to never give routine vaccinations, CCMG may not be a good fit for you.

  • Medical Home

    We work hard to provide comprehensive medical care and serve as your medical home. If able, please contact our office FIRST before seeking specialty care or urgent care at another facility. This enables us to refer you to the most appropriate specialist and help to coordinate your care.


    Whenever you do see a specialist, please request a report be sent directly to our office so we may stay informed and have the most up-to-date information in your medical record.

  • Technology

    Our practice strives to improve efficiency through use of technology. You will be encouraged to consult our website, register for and use our patient portal, and effectively use automated reminders for appointments and for routine care/immunizations that are due.

  • Scheduling

    To help provide timely service to our patients, we prefer to schedule all visits. We make every effort to meet the needs of our patients, offering both routine well visits and sick visit appointments. 


    Saturday office hours are reserved for newborn infants and acute care visits. 


    Walk-in appointments  are discouraged and are scheduled in the first available sick visit appointment slot.  Please note there is an additional fee for walk-in appointments and appointments seen after scheduled business hours or on holidays/Saturdays.

     

    Please note that most acute care visits are slotted for 10-15 minutes and well care visits for 15-30 minutes.  If you anticipate being unable to cover your concerns in this time frame, alert the scheduler when making the appointment.

  • Appointments

    For acute care, call the office to speak with staff and schedule an appointment if appropriate. Sometimes an appointment may not be needed, and triage staff will advise you regarding your issue or check with the physician and call you back.  If our triage staff is not immediately available, you will be asked to leave a message and you will receive a return call.  If you are confident you would like your child seen, please indicate this to the scheduler upon calling to secure an appointment.


    Well care visits help keep your child healthy by providing you with current information on issues such as nutrition, behavior, development, safety and vaccinations.  We recommend that you call our office 3 months in advance to schedule an appointment with your child's provider on the date that best fits your needs.   


    Please note that most acute care visits are slotted for 10-15 minutes and well care visits for 15-30 minutes.  If you anticipate being unable to cover your concerns in this time frame, alert the scheduler when making the appointment.

  • Timeliness & Missed Appointments

    Our practice makes every effort to run on time with appointments, as we believe everyone’s time is equally valuable. As a courtesy, we will remind you of your upcoming appointments via phone/text message/email.


    We ask that you arrive 10 minutes before your scheduled appointment time and be prepared to provide your insurance card, ID, and copay.  


    We understand sometimes things happen beyond your control that may cause you to be late.  However, we reserve the right to ask you to reschedule if you arrive late for your appointment.


    Missed Appointments: Broken appointments represent a cost to us, to you, and to other patients who could have been seen in the time set aside for you. We reserve the right to charge a fee for canceled or missed appointments. We request 24 hours notice for cancellation of appointments.


    A fee may be charged for a second missed appointment. The third consecutive missed appointment will result in discharge from the practice.

    For new patients, a fee may be charged if the FIRST appointment is missed.

  • After Hours

    We have a triage service and physician on call 24/7/365. When our office is closed overnight and you have an urgent issue that cannot wait until morning, you can call our main number and receive advice regarding your child's problem.

  • Insurance

    Make sure we participate with your insurance plan. 


    It is YOUR responsibility to know the limits and coverage of your particular health insurance policy, to show your cards to us at each visit, and be prepared to pay any copays or fees at the time of service.


    Our billing staff will do their best to assist you with insurance questions; however, if you have questions about your coverage, it is best check with your specific insurance company. Our office is required bill your health plan based on federal guidelines and the actual services provided.

  • Billing

    Insurance copays are expected to be paid at the time of service and account balances will be collected. If you are unable to comply, you must speak with the billing department prior to the visit to set up a payment plan.
