Are You Expecting?
Congratulations!
Starting a family or welcoming another child into your family is a wonderful experience and truly a gift. Along with the beauty and joy of pregnancy, delivery, and then coming home, there are bound to be times when you have questions about how to care for your baby and family.
We are here for you and your family.
We are excited and honored that you are considering us for your child’s care. It is always special for us to be invited by new parents to join in the care of a family member. The continuity of care and the amazement of watching a baby grow and develop into a wonderful child is one of the primary reasons that we all feel privileged to work in pediatrics.

What To Bring
- Any paperwork from the hospital, including hearing screen results and immunization records.
- Any available insurance information (often only the parent’s insurance is available at the first visit).
- Plenty of diapers and an extra set of clothes.
- Completed new patient forms done prior to the visit to help make the appointment as smooth as possible.
- Insurance companies mandate you add your newborn to your health insurance plan within 30 days of birth or they will not cover any services. You may need your child's social security number from their birth certificate, once issued. We recommend you prioritize these tasks to avoid non-covered bills from hospitals and doctors.
Before Your First Appointment
- A couple of weeks before delivery, call the office to let us know you are expecting.
- Download and fill out the new patient paperwork ahead of time.
- When the happy day has arrived, tell the hospital staff that you have chosen
CCMG
as your pediatric practice.
