We Accept Most Insurance Carriers

We accept all major PPO insurance carriers:

• Aetna
• Anthem Blue Cross
• Blue Shield (see below)
• Cigna
• United Healthcare
• Tricare

Please bring a physical copy of your current insurance card and payment at every office visit. Please check with your insurance company to confirm our office is covered by your plan prior to making an appointment.  Please provide your insurance information at the time of scheduling. Payment is due at the time of service.

As of October, 2025 Dr. Rubin is no longer an in-network provider for Blue Shield plans due to an administrative ruling that was not related to her practice of medicine. Our office plans to appeal this decision annually until Blue Shield reinstates her.

We also accept cash payment for visits. We do not accept HMO plans or Medi-cal programs at this time. Our standard office procedure is that office visit copayments be paid at the time medical care is received. If you are a cash-paying patient, we require all services be paid for in full at the time of your visit.

We believe it is more convenient to pay your statement balance online through our patient portal.  We will send a paper statement if you opt out of this choice.

If payment is not received at the time due, we will charge your credit card on file.
